چکیده

The anatomic location of atrioventricular (AV) bypass tracts (or accessory pathways) is variable, but these generally traverse the tricuspid or mitral annulus and insert into the atrium and ventricle near the AV ring. There is only 1 case report of an accessory pathway ablated in the left coronary cusp (LCC), and this pathway had bidirectional conduction, was not decremental, and was associated with an orthodromic tachycardia. We report here the first case of an antegrade slowly conducting, decremental, accessory pathway that was also successfully ablated in the aortic LCC. This pathway generated antidromic tachycardia having a QRS morphology mimicking those seen in outflow tract ventricular tachycardia.

Mahaim fibre coexisting with anomalous origin of the left main coronary artery from the right aortic sinus and an interarterial course.

We present the case of a 45-year-old female with a normal heart, who exhibited an episode of a broad QRS tachycardia that was initially suggested to be sustained ventricular tachycardia. Coronary angiography showed the left main stem originating from the right aortic sinus with an interarterial course between aorta and right ventricular outflow tract. Electrophysiological study revealed the pre...

Characterization of Aortic Valve Closure Artifact During Outflow Tract Mapping: Correlation With Hemodynamics and Mechanical Valves.

BACKGROUND Premature ventricular contractions originating in the left ventricle outflow tract represent a significant subgroup of patients referred for catheter ablation. Mechanical artifacts from aortic valve leaflet motion may be observed during mapping, although the incidence and characteristics have not been reported. METHODS AND RESULTS Twenty-eight consecutive patients with left ventric...

Radiofrequency ablation of ventricular premature contraction originating from a native coronary cusp after transcatheter aortic valve replacement

We describe a case of radiofrequency ablation of ventricular premature contraction (VPC) originating from the left ventricular outflow tract after transcatheter aortic valve replacement. The VPC origin was the native aortic valve annulus between the left and right coronary cusps. Radiofrequency ablation was successfully performed by manipulating the ablation catheter from the gap between the si...

Repetitive monomorphic ventricular tachycardia originating from the aortic sinus cusp: electrocardiographic characterization for guiding catheter ablation.

OBJECTIVES We sought to investigate the electrocardiographic (ECG) characteristics for guiding catheter ablation in patients with repetitive monomorphic ventricular tachycardia (RMVT) originating from the aortic sinus cusp (ASC). BACKGROUND Repetitive monomorphic ventricular tachycardia can originate from the right ventricular outflow tract (RVOT) and ASC in patients with a left bundle branch...

Ventricular arrhythmias arising from the left ventricular outflow tract below the aortic sinus cusps: mapping and catheter ablation via transseptal approach and electrocardiographic characteristics.

BACKGROUND Ventricular arrhythmias (VAs) originating from the anterosuperior left ventricular outflow tract (LVOT) represent a challenging location for catheter ablation. This study investigates mapping and ablation of VA from anterosuperior LVOT via a transseptal approach. METHODS AND RESULTS This study included 27 patients with symptomatic VA, of which 13 patients had previous failed ablati...
